29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  a) A trench which measured five (5) feet nine (9) inches in depth was not provided with an adequate protective system, thereby exposing the employees to cave in hazards.    Doherty, Giannini, Reitz Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 313941049, citation number 1, item number 1 and was affirmed as a final order on February 28, 2011 with respect to a workplace located at 1306 S. Washtenaw Ave., Chicago, IL  60608.  Doherty, Giannini, Reitz Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 314040452, citation number 1, item number 1 and was affirmed as a final order on January 27, 2011 with respect to a workplace located at 300 Riverside Ave., St. Charles, IL  60174.  Doherty, Giannini, Reitz Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 314038381, citation number 1, item number 4 and was affirmed as a final order on October 28, 2010 with respect to a workplace located at Park Avenue West of IL Rt. 47, Sugar Grove, IL  60554  Doherty, Giannini, Reitz Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 313936478, citation number 1, item number 1 and was affirmed as a final order on August 24, 2010 with respect to a workplace located at 3230-3232 W. 111th Street, Chicago, IL  60655.
