29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    a) Esmark Excalibur, LLC / 6103 US Highway 6, Linesville, PA 16424: On or about July 1, 2014 there was a long piece of wood that extended past the top of the door frame and down through the doors handle bar.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Esmark Excalibur, LLC / 6103 US Highway 6, Linesville, PA 16424: On or about July 1, 2014 a Miller welding machine on the West wall was plugged into a knockout style receptacle outlet. The knockout style receptacle outlet was not designed for portable applications in that it was connected to the end of a flexible cord and capable of being moved around the floor.
