29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)  Production Floor;  On July 23, 2014 and at times prior to; the employer exposed his employees to potential hazardous energies in that employees were allowed to work on the 300 ton automatic press machine, among others, which has multiple energy sources such as 230 VAC electrical and 90 psi pneumatic pressure, without those employees having a written machine specific lock out procedure to ensure that those energy sources were controlled while the employees were performing maintenance or tool changes at least weekly.      (b)  Production Floor;  On July 23, 2014 and at times prior to; the employer exposed his employees to potential hazardous energies in that employees were allowed to work on the CS 250 ton press machine, among others, which has multiple energy sources such as 480 VAC electrical and 250 tons of hydraulic pressure, and the weight of the press that is not blocked, without those employees having a written machine specific lock out procedure to ensure that those energy sources were controlled while the employees were performing maintenance or conducting tool changes at least weekly.
