The employer has failed to submit abatement certification and documentation within ten days of the abatement date, September 29, 2014, for a violation of 29 CFR 1926.1053(b)(6), issued as citation 1, item 2, and described as follows:  Located at 5960 Carrbridge Court in Galloway, Ohio, employees were using an extension ladder to gain access to the second floor roof from the first floor roof. The ladder was placed on an angled surface and was unsecured, thereby exposing employees to a serious fall hazard.  c.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, September 29, 2014, for a violation of 29 CFR 1926.1053(b)(22), issued as citation 1, item 3, and described as follows:  Located at 5960 Carrbridge Court in Galloway, Ohio, employees were carrying large pieces of material up an extension ladder and not maintaining full control while climbing, thereby exposing employees to a serious fall hazard.
