29 CFR 1926.102(a)(1): Eye or face protective equipment were not used when machines or operations presented eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) At the residential construction site located at 8215 NE 74th Terrace, Kansas City, Missouri: Employees were exposed to struck-by hazards. Employees engaged in framing activities, using a pneumatic nail gun, were exposed to struck by hazards in that eye protection was not worn.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  At the residential construction site located at 8215 NE 74th Terrace in Kansas City, Missouri: Employees engaged in residential framing activities were exposed to fall hazards of approximately 10 foot from the second level of the structure to the floor of the house interior below. Fall protection was not provided as employees walked the first floor ceiling joists.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) At the residential construction site located at 8215 NE 74th Terrace, Kansas City, Missouri: An employee engaged in residential framing was exposed to fall hazards when the portable step-ladder he stood on was closed, not in the locked open position, and thus used other than it's designed purpose.
