29 CFR 1926.1101(k)(2)(ii)(A): Building and/or facility owners failed to notify prospective employers applying for or bidding for work whose employees can reasonably be expected to work in or adjacent to areas containing asbestos containing material or presumed asbestos containing material of the presence, location and quantity:    1) Horizon Group Management, LLC dba Horizon Realty Group is the owner of the building at 1611 Chicago Avenue, Evanston, IL and did not notify the general contractor who bid for work at the work site in writing or personal communication of the presence, location, and quantity of asbestos containing material (ACM) or presumed asbestos containing material (PACM) at the basement work site. A superintendent of the general contractor was onsite daily during demolition work in the basement where ACM was present.     In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.     HORIZON GROUP MANAGEMENT, LLC dba HORIZON REALTY GROUP WAS PREVIOUSLY CITED FOR THIS VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.1101(k)(2)(ii)(A), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 691538, CITATION NUMBER 1, ITEM 3, ISSUED ON MARCH 21, 2013, WITH RESPECT TO A WORKPLACE LOCATED AT 5050 N. SHERIDAN, CHICAGO, ILLINOIS.     THE FINAL ORDER DATE FOR INSPECTION NUMBER 691538 WAS APRIL 9, 2013.
