29 CFR 1910.303(b)(8)(iii): Electric equipment provided with ventilating openings was not installed so that walls or other obstructions do not prevent the free circulation of air through the equipment:  On or about July 29, 2014, and at times prior thereto the three-phase, 480V/208V Hammond Power Solutions Inc. Transformer Cat No. NMK150KB was installed within 3-inches of the metal wall of the building. The installation did not allow for the minimum ventilation clearance of 6-inches as required by the manufacturer to prevent them from overheating and exposing employees to fire hazards.
