29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 08/08/14 at 269 Hidden Valley Road, Gates, New York; employer failed to protect two employees against fall hazards when they were installing roofing shingles, on the front roof near the peak, at an estimated height of 10 feet at the eave and not more than 25 feet at the peak with an 8/12"  pitch.  Two slide guards were installed on the rake edge and one slide guard was installed at the bottom of the roof edge.          b) On or about 08/08/14 at 269 Hidden Valley Road, Gates, New York; employer failed to protect two employees from fall hazards when they were installing roofing shingles, on the back roof near the peak, at an estimated height of 10 feet at the eave and not more than 25 feet at the peak with an 8/12" pitch.  Two slide guards were installed at a midway point across the roof and one slide guard was installed at the lower portion near the eave.      NO ABATEMENT CERTIFICATION REQUIRED
