29 CFR 1917.45(e)(2): Crane hooks were used without safety latches or other means to prevent accidental load disengagement:  Samson Yard:  On or about August 11, 2014 and at times prior thereto, the hooks on the 4-way lifting bridle utilized by the Manitowoc 4000W, Equipment Number SC403 to move marine containers were not equipped with safety latches.  This condition exposed employees to struck-by hazards in the event of load disengagement.

This condition exposed the employee to struck-by hazards.

Oxygen cylinder was adjacent to wood stairwell structure. This condition exposed employees to smoke-inhalation injuries.
