29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed:  a.  United Community Bank, 3510 Henderson Mill Road, Chamblee, GA - Roofing employees were exposed to a fall hazard.  Employees took apart a 30' aluminum portable extension ladder and used one section of the ladder 15' as a crawling chicken board ladder, which was used as steps to gain access to an upper level roof area, on or about 09/03/14.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a.  United Community Bank, 3510 Henderson Mill Road, Chamblee, GA (Jobsite) - Roofing employees were exposed to a fall hazard.   Employees were not utilizing a means of fall protection, and/or tied off to an anchorage point while walking and working approximately 20 feet above ground level on the elevated roof with a 10:12 slope pitch. The employees were installing new roof materials such as, felt paper and roof shingles on the elevated roof,  on or about 09/03/14.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
