29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.    (a) Roof of building # 2, 740 Sidney Marcus Blvd, Atlanta, GA: Employees were exposed to fall hazards while improperly using a harness and carabineers as a positioning belt with a rope grab located at the wearer's waist.  The employees were on an 8:12 pitched roof at a height of approximately 28 feet from the eave to the lower level.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a) At the roof of building # 2 located at 740 Sidney Marcus Blvd, Atlanta, GA: Employees using Pneumatic Hitachi Nail Guns (Model: NV 45AB2) were not protected from eye injuries while installing felt and shingles on the roof.      Home Live Roofing, LLC, was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.102(a) (1) which was contained in OSHA Inspection Number 945480, Citation #1, Item #1, issued on 12/06/2013 with respect to a work place located at 945 Crestmark Blvd, Bldg. # 400, Lithia Springs, GA 30122 and became a final order on 12/30/2013.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) Roof of building # 2 located at 740 Sidney Marcus Blvd, Atlanta, GA: Employees installing felt and shingles on an 8:12 pitched roof at a height of approximately 28 feet were not using proper fall protection system.      Home Live Roofing, LLC, was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b) (13) which was contained in OSHA Inspection Number 945480, Citation #1, Item #3, issued on 12/06/2013 with respect to a work place located at 945 Crestmark Blvd, Bldg. # 400, Lithia Springs, GA 30122 and became a final order on 12/30/2013.
