29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:   a)  Main shop area to left of office door, 30 Georgetown Road, Canonsburg PA 15317:  Observed on or about November 03, 2014, a horizontal lathe (unknown manufacturer) used to cut zinc samples was not equipped with a chuck guard.  Portions of the chuck jaws protruded approximately 1 inch beyond the chuck.  Employees using the lathe were exposed to exposed to caught-by hazards created by the rotating lathe chuck.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:                                                                                                      a) Warehouse area, 30 Georgetown Road, Canonsburg PA 15317:  Observed on or about November 03, 2014, a black flexible cable (240 Volt, 3 phase) was not equipped with proper strain relief.  A portion of the outer cover of the cable was  worn away where the cable enters the plug.  Employees using the cable were exposed to the hazard of electric shock.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and/or cables were run through doorways, windows, or similar openings:  a) Warehouse area, 30 Georgetown Road, Canonsburg PA 15317:  Observed on or about November 03, 2014, a yellow flexible cord (110 Volt) was run beneath a bay door in the warehouse area of the facility.  The cord was exposed to wear and tear from the opening and closing of the bay door.  Employees using the cord and employees using the bay door were exposed to the hazard of electric shock.   b) Warehouse area, 30 Georgetown Road, Canonsburg PA 15317:  Observed on or about November 03, 2014, a black flexible cable (240 Volt, 3 phase) was run beneath a bay door in the warehouse area of the facility.  The cord was exposed to wear and tear from the opening and closing of the bay door.  Employees using the cord and employees using the bay door were exposed to the hazard of electric shock.
