29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts: a. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Jovil Universal" model SMC-2E, serial number 2009012 winding machine was guarded during operation. b. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Guangri electric Machinery" model JG-6204, serial number 00-6204-D822C-6 winding machine was guarded during operation. c. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Guangri electric Machinery" model JG-4163, serial number 2000-4163-C92376 winding machine was guarded during operation. d. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Guangri electric Machinery" model JG-7204, serial number 7204-7-109-168 winding machine was guarded during operation. e. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "V & C Standard" model SVE-3, serial number 2030310 winding machine was guarded during operation. f. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Guangri electric Machinery" model JG-8204, serial number 8204-4410 winding machine was guarded during operation. g. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Guangri electric Machinery" model JG-8204, serial number 8204-0111 winding machine was guarded during operation. h. On or about December 12, 2014 employees working in the winding department were exposed to caught-in hazards where the employer did not ensure that moving parts of the "Guangri electric Machinery" model JG-8554T, serial number 2000-8554-01-6 winding machine was guarded during operation.

On or about December 12, 2014 adjacent to the low voltage molding department employees were exposed to electrical hazards where the employer used flexible cord as a substitute for fixed wiring to provide power to electric motors used to rotate drums containing Stobicast M 615.02-B and Cridel resin.

On or about December 12, 2014 adjacent to the low voltage molding department employees were exposed to electrical hazards where the employer used flexible cord attached to a metal storage rack using cable ties to provide power to electric motors used to rotate drums containing Stobicast M 615.02-B and Cridel resin.
