29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about November 20, 2026, the employer did not ensure that the Winder #1 ULTI- M.E.T. (model no. 400, serial no. 97387) was provided with adequate guarding to protect employees from injury from the rotating face plate and attached mounting fixture during the operation of the unit creating an entanglement hazard. (b) On or about November 20, 2026, the employer did not ensure that Winder #2 was provided with adequate guarding to protect employees from injury from the rotating face plate and attached mounting fixture during the operation of the unit creating an entanglement hazard. (c) On or about November 20, 2026, the employer did not ensure that Winder #3 was provided with adequate guarding to protect employees from injury from the rotating face plate and attached mounting fixture during the operation of the unit creating an entanglement hazard. (d) On or about November 20, 2026, the employer did not ensure that Winder #4 was provided with adequate guarding to protect employees from injury from the rotating face plate and attached mounting fixture during the operation of the unit creating an entanglement hazard. (e) On or about November 20, 2026, the employer did not ensure that Winder #5 was provided with adequate guarding to protect employees from injury from the rotating face plate and attached mounting fixture during the operation of the unit creating an entanglement hazard.
