29 CFR 1904.32(a)(1):  The employer did not review the OSHA 300 Log or equivalent to verify that the entries are complete and accurate and any deficiencies identified corrected.  (a) D.C. Operations - On or about February 1, 2015, employee injuries recorded on the 2014 300 log were not reviewed by the employer to verify that the entries were complete and accurate and deficiencies corrected, in that columns F and M1 were not accurately completed.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Operations -  On or about April 30, 2015, the employer's certified 2014 300A Summary of Work-Related Injuries and Illnesses was not reviewed extensively to ensure that entries were correct and complete in that columns H, I, J, K, L, M1, M3, M5 and M6 were not calculated correctly.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
