29 CFR 1910.179(b)(6)(ii): Where passageways or walkways were provided, obstructions were so placed that the safety of personnel was jeopardized by movements of crane(s)   (a) On or about January 7, 2015, the employer exposed employees to a falls and struck-by hazards due to operating the scissor lift in the passageways, which obstructed the bridge of the overhead crane. 29 CFR 1910.179(l)(3)(ii): Adjustments on crane(s) were not maintained to assure correct function of (add one or more of the following)   (a)  On or about January 7, 2015 and at times prior, the employer did not ensure adjustments or repairs was conducted on the following area of overhead bridge crane:  1. Preventive maintenance was not conducted on the hoist braking system. The hoist brake has damaged threads which prevented taking the brake completely apart.   2. Preventive maintenance was not conducted on wireless control pendent knob. The start knob was broken in which the employee were utilizing a screw driver to start the crane with the wireless control pendent.
