29 CFR  1910.147(d)(3):  All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:  (a)  On or about November 2, 2021, Hartung Glass Industries did not perform the lock-out/tag-out procedures for the Forvet CNC milling machine, serial # 423/08,  by isolating the electrical and pneumatic power sources as required by the company's lockout tagout procedures, located in Hartung Industries Safety Program # 25, resulting in an employee suffering serious injuries while performing maintenance on the machine following a machine malfunction.
