29 CFR 1910.147(a)(3)(i): The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees. On or about January 4, 2015, the employer did not ensure that the Tray Wash Return Conveyor was locked out before allowing an employee to work on a chain and sprocket, exposing employees to a struck by hazard.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about January 4, 2015, the employer failed to provide training for recognizing hazardous energy and the methods and means necessary for isolating the equipment for the employee conducting preventive maintenance on the Tray Wash Return Conveyor, exposing the employee to amputation hazards.
