29 CFR 1926.760(a)(1):  Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet* (4.6m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:  (a)  Commercial Tire - On or about December 22, 2014, and at times prior thereto, employees were not protected from exterior falls while laying roof decking, which exposed them to the hazard of falling approximately 18 feet ground to eave.  Note: Abatement certification IS required for this item.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:  (a)  On or about December 22, 2014, and at times prior thereto, employees demonstrated the inability to recognize and identify fall hazards and mitigate them with correct procedures to implement for fall protection.  Note: Abatement certification IS required.
