29 CFR 1926.951(a)(1)(i): Rubber protective equipment was not provided to employees who perform overhead power line installations in accordance with the provisions of ANSI J6 series:  Or in the Alternative:   29 CFR 1910.269(q)(1)(iii): When a pole was set, moved, or removed near an exposed energized overhead condition, the employer did not ensure that each employee wear electrical protective equipment or use insulated devices when handling the pole:  a. On or about February 12, 2015, at the jobsite, a employee spotting a 50-foot wooden pole that was being installed in the ground by a digger derrick was shocked when the pole was energized due to coming into contact with the top phase of a 13.2kV overhead power line. The employee was not wearing any personal protective equipment such as hot gloves, sleeves or EH rated dielectric overshoes, resulting in a hospitalization.

29 CFR 1910.269(p)(4)(iii)(A): The energized lines exposed to contact were not covered with insulating protective material that will withstand the type of contact that might be made during the operation.  a. On or about February 12, 2015, three employees were replacing an overhead power line utility pole, the pole was moved and came into contact with the existing unprotected power lines electrically shocking one of the employees, the existing power lines were not covered with an insulating protective material that could withstand the type of contact that might be made during the operation, the employees were exposed to 13.2kv electrical shock hazard or electrocution hazards.
