OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  F/V Aleutian Sable, Coastal Dock; on or about 6 February 2015, and times prior thereto; four (4) instances were noted where electrical hazards existed:           (a) Bow Storage area:  a 208 volt electrical panel was missing a cover and dead front.      (b) Bow storage area; a 100 foot yellow/blue extension cord cut insulation and wiring taped together with black electrical tape.          (c) Wheel house stairs:  a 120 volt light bulb was unguarded from damage and had exposed wiring.       (d) Lower state rooms:  a 50 foot orange extension cord was routed down the hallway and into a stateroom, through doorways.   These conditions exposed employees to electrical shock and electrical burn hazards and electrocution.

29 CFR 1915.73(g): Gratings, walkways, and catwalks, from which sections or ladders have been removed, were not barricaded with adequate guardrails.  Aleutian Sable, Coastal Dock, Weather deck;  On or about 6 February 2015 and times prior thereto, a 6" by 48" deck planking was removed on the main deck.  This condition exposed employees to a tripping hazard.

29 CFR 1915.81(a)(1): The employer did not maintain good housekeeping practices to eliminate hazards to employees.  F/V Aleutian Sable, Coastal Dock, Bow storage area:  On or about 6 February 2015 and times prior thereto, equipment and supplies were stored impeding access to the forward electrical panels and also creating tripping hazards.  This condition exposed employees to tripping hazards and delays in accessing electrical panels resulting in smoke inhalation.

F/V Aleutian Sable, Coastal Dock, Factory Deck:  On or about 4 February 2015 and times prior thereto, the Ryan  and Baader 415  Header machines had their cutting saws removed and were not locked out.  This condition exposed employees to an amputation hazard.

(a)  F/V Aleutian Sable, Coastal Dock, Weather deck:  On or about 6 February 2015 and times prior thereto, the 91" x 91" hatch was open without railings installed.  This condition exposed employees to a 118" fall hazard.  (b)  F/V Aleutian Sable, Coastal Dock, Aft deck:  On or about 6 February 2015 and times prior thereto, the 20" x 24" hatch was open without railings installed.  This condition exposed employees to a 91" fall hazard.  (c)  F/V Aleutian Sable, Coastal Dock, Main deck:  On or about 6 February 2015 and times prior thereto, the 20" x 24" hatch was open without railings installed.  This condition exposed employees to a 118" fall hazard.  (d) F/V Aleutian Sable, Coastal Dock, Lower bow storage access ladder:  On or about 6 February 2015 and times prior thereto, the 31" x 36" lower bow storage deck hatch was unguarded without railings installed.  This condition exposed employees to a 91" fall hazard.

This condition exposed employees to a struck by hazard.
