29 CFR 1915.89(g)(1):   The employer did not ensure that the authorized employee, or the primary authorized employee in a group lockout/tags-plus application, continues verifying deenergization and isolation while servicing the machinery, equipment, or system:       F/V Aleutian Sable:  On September 4, 2018, the employer did not verify continuing deenegization and isolation during maintenance of the ammonia refrigeration system.  This condition exposed employees to inhalation and skin burns from anhydrous ammonia and resulted in in-patient hospitalization and follow-up care.

This condition limited employee information regarding the failure of the lockout/tags plus program and mechanical systems involved in the failure and continues to expose employees to inhalation and skin burns from anhydrous ammonia.
