F/V Alaska sprit, OSI Dock; on or about 11 February 2015, and times prior thereto; Two (2) instances were noted where electrical hazards existed:           (a) Factory area, starboard companionway:  a 208 volt electrical panel missing cover and dead front exposing wires.      (b) Port companionway; a 50 foot yellow extension cord missing a ground pin.             These conditions exposed employees to electrical shock and electrical burn hazards.
