(b) At Executive Coach on or about March 26, 2015 a Millermatic arc welder Model 251 had a set of valves attached to a compressed gas cylinder used for MIG welding that was damaged and the welder had not been removed from service or the gauge repaired or replaced.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
