29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a) Facility, 3866 Millers Run Road, McDonald, PA:  On or about March 18, 2015 employees operate the Case and JLG powered industrial trucks and no training and evaluation had been provided to ensure they were competent to operate each piece of equipment safely.
