29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  A). On or about 03/25/15, at the site of 212-35 86th Road. Queens Blvd, NY  Employees working at the site were in danger of head injury from falling objects such as, but not limited to tools and plywood from other employees who working on above the attic floor. Protective helmets were not provided.  ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

Queens Village, NY  Employees installing plywood on the attic floor were exposed to the hazard of falling approximately 20 ft. to the ground below.  ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

On or about 03/25/15, at the site of 212-35 86th Road. Queens Village, NY   Employees using the A-frame ladder on the second floor for gain access to the attic floor. There was a break in elevation of 48 inches from the top of the ladder to the attic floor.   ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
