29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    (a). At the 5th floor North side of the building, the top cable of the guardrail system failed breaking away from the clamps causing injuries to an employee who fell to the lower level roof top, on or about March 26, 2015.

29 CFR 1926.502(b)(6): Guardrail systems shall be so surfaced as to prevent injury to an employee from punctures or lacerations, and to prevent snagging of clothing.  (a).  At the 5th floor north side of the building, the wire cable used for the top rail of the cable guardrail system was frayed at the left hand side exposing employees to punctures and lacerations, as observed on or about March 31, 2015.
