29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to employees. Such facilities were not in near proximity to the worksite and were not equipped as to enable employees to remove such substances. Employer did not provide adequate washing facilities for employees engaged in the application of paints and coating. This conditions exposed employee to adverse health hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. Employees working on elevated surfaces were using a scissor lift to gain access to the roof. This condition exposed employee to fall hazards exceeding 10 feet.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. Employees were using a mobile scaffold to gain access to the roof of the building without having a above or below a point of access. This condition exposed employee to fall hazards exceeding 10 feet.
