29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On or about January 23, 2024, the employer did not submit documentation and Certification of Corrective Action for citation 1 item 2 issued on November 27, 2023, with an abatement due date of December 14, 2023.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On or about October 30, 2023, the employer did not ensure that employees working on a supported scaffold were protected from falls of over 10 feet. This condition exposed employees to fall hazards.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:  On or about October 30, 2023, the employer did not ensure employees working on a supported scaffold retained the proficiency on the use of fall protection while working on a scaffold. This condition exposed employees to fall hazards of over 10 feet.
