29 CFR 1910.134(c)(1):  In any workplace where respirators were necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use:  (a) Foster Supply Inc.:  The employer has not established and implemented a written respiratory protection program to protect employees from respiratory hazards such as but not limited to, respirable silica dust.  The employer requires the use of a 3M 8293 filtering facepiece particulate respirator during grinding of precast concrete pieces, as determined on April 15, 2015.    Abatement Certification Required for this Item.

The employer has not provided the employee with a medical evaluation to determine the employees ability to wear the respirator prior to required respirator use, as determined on April 15, 2015.
