29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  a) Construction site, Warrington Point Lot 10 thru 14 - On or about April 27, 2015, An employee was observed caulking around windows while working from a pump jack scaffold that did not have mid-rails/netting installed to the back side, or protection at the exposed end, exposing the employee to serious injury in the event of a fall.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Construction site, Warrington Point Lot 10 thru 14 - On or about April 27, 2015, An employee was observed installing fascia cladding while working from the roof of the structure without being protected from falling more than 20 feet to the ground, exposing the employee to serious injury in the event of a fall.
