29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers.  Roof, Atlas Box and Crate Warehouse Addition Project, 223 Worcester-Providence Turnpike: On or about May 2, 2015: employees were working on a roof over 28 feet above the next lower level with floor holes over 5 inches in diameter or width which did not protect employees from tripping in or stepping into or through such holes.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
