29 CFR 1926.800(j)(1)(i)(B): Where monitoring of airborne contaminants was required, the competent person did not make a reasonable determination as to which substances to monitor and how frequently to monitor taking into consideration work practices and jobsite conditions:  Inside tunnel: On or about May 6, 2015, and at times prior thereto, employees were exposed to the chemical hazards of Carbon Monoxide, Diisocyanates, and low Oxygen levels without the competent person being trained to make a reasonable determination of the chemical jobsite hazards.
