29 CFR 1926.800(j)(2)(i): For potentially gassy or gassy operations, a test for oxygen content was not conducted in affected underground work areas and work areas immediately adjacent to such areas at least at the beginning and midpoint of each shift: On or about July 13, 2016 employees tasked with performing welding operations inside a tunnel did not take air monitor reading prior to beginning the work exposing the workers to asphyxiations.
