29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): A) Polyfill, LLC, located at 960 North Vandemark Road, Sidney, OH 45365: On or about May 7, 2015, the employer failed to ensure that open-sided platform/catwalk, located at the top of the mezzanine on Line # 2, was equipped with standard railing in accordance with 29 CFR 1910.23(e). Employees were exposed to a fall hazard of approximately 10 feet from the floor of the platform/catwalk to the grade floor below.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section A) Polyfill, LLC, located at 960 North Vandemark Road, Sidney, OH 45365: On or about May 11, 2015, the employer did not ensure that employees utilized established energy control procedures to control hazardous energy of the calendar rolls on Line # 2. Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic before performing activities such as, but not limited to, threading plastic through four calendar rollers inside the calendar house. Employees were exposed to hazards associated with unexpected start-up of the rollers while all hazardous energy sources were not controlled prior to conducting work.
