Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     On or about May 18, 2015, an employee was hospitalized due to an injury from a forklift accident where the employee sustained an injury (broken right arm) that resulted in in-patient hospitalization, along with days away from work, and the employer failed to report the amputation to OSHA within 24 hours.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
