29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about March 2, 2021, the employer did not report to OSHA within 24 hours a work-related hospitalization of an employee who fell to a concrete floor injuring his head.  The employee was working from an 8 foot step ladder adjusting a wall mounted video camera that was located approximately 10 feet above the warehouse floor when he fell.  The hospitalization was reported to OSHA on March 10, 2021.   There is no abatement documentation or certification required for this item.
