29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system:    (a)   Douglass Colony Group, Inc. at 10701 N. Pecos St., Northglenn, CO 80234:  On or before April 10, 2015 the exposing employer did not ensure that employees were protected from injury, in that two employees installing EPMD on the perimeter edge on a flat roof of a two-story apartment building were not utilizing a fall protection system.  This condition exposed the employees to a fall of approximately 25 feet.

29 CFR 1926.502(f)(2)(ii):  The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface:      (a)   Douglass Colony Group, Inc. at 10701 N. Pecos St., Northglenn, CO 80234:  On or before April 10, 2015 the exposing employer did not ensure that the lowest point of the warning line system, which was installed on the flat roof of a two-story apartment building, was not less than 34 inches.  The warning line had fallen down in the area where two employees were working.  This condition exposed the employees to a fall of approximately 25 feet.
