On or about May 27, 2015, the employer did not ensure employees were utilizing the procedures for the safe application, usage, and removal of energy controls during die changes, material changes, and the changing of punches inside the point of operation of a Niagara 50 ton, part revolution mechanical power press, Serial No. 30223, known as the Mini Line #4. Energy sources include, but not limited to, electric, pneumatic power, and mechanical power. b. On or about May 27, 2015, the employer did not ensure energy control procedures were developed for the safe application, usage, and removal of energy controls during placement of rolls of material on the machine slide, stringing material through the machine, and making adjustments to the rolls and material of a Highway Handyman In-line Applicator machine, Serial No. 1304-1192. Energy sources include, but not limited to, electric and pneumatic power.
