29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)Villa Construction LLC at 4435 Walden Court, Denver, CO 80249:  On and before May 30th, 2015, the employer did not ensure the use of safety glasses when employees were using or working near pneumatic nail guns to side the single family home under construction.  The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a)Villa Construction LLC at 4435 Walden Court, Denver, CO 80249:  On and before May 30th, 2015, the employer allowed the use of two sets of pump jack scaffolds that were missing the mid-rails and end-rails, while the employees were siding a two-story single family home under construction. The condition exposed employees to fall hazards of up to 20 feet.

The condition exposed employees to fall hazards of up to 13.7 feet.
