29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Facility, 400 9th Street, Monaca, PA:  On or about 06/07/15 machine guarding was not provided on the blank side of the shop 1 glass forming machine to prevent employees from contact with moving parts during swabbing operations.  An employee had their left hand inside the blank to swab it when the machine cycled.  Molten glass came down from the funnel and the baffles came down causing crushing injuries and 3rd degree burns to the employee's left hand.
