29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):               a)  Finishing Department, 400 Ninth Street, Monaca, PA 15061:  On or about August 11, 2015, the south side (opposite side of the operator's station) of the #7 bulk palletizer (Shop 7 bulker, model number 1200, serial number 7992-f) was not guarded to prevent employees from having any part of their body in the danger zone during operating cycles.  Employees who operated or serviced the #7 bulk palletizer were exposed to caught-by hazards.    b)  Finishing Department, 400 Ninth Street, Monaca, PA 15061:  On or about August 11, 2015, the north side (opposite side of the operator's station) of the #3 bulk palletizer (Shop 3 bulker, model number 3002D, serial number 2854481349) was not guarded to prevent employees from having any part of their body in the danger zone during operating cycles.  Employees who operated or serviced the #7 bulk palletizer were exposed to caught-by hazards.
