29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms.     (a) Location - 2 Reed Street, Haverhill, MA:  On or about 7/8/2015, employees working at a height of 18 feet 4 inches above the ground on a pump jack scaffold platform were not protected from falling by means of a complete guardrail system.
