29 CFR 1910.178(a)(4): Capacity, operation, and maintenance insturction plates, tags or decals of the powered industrial truck were not changed accordingly.  Laydown Yard, 115 Coburn Avenue, Gardner, MA: On or about September  16, 2016 employees were permitted to use a JLG 10054 Skytrak Telescoping Lift with fork extensions without insuring that capacity, operation and maintenance tags or decals were changed to reflect new safe operation instructions.
