Green Bay, WI: On July 13, 2015 employees were installing a new concrete storm sewer manhole in an unprotected 6 feet deep and greater excavation exposing employees to side wall collapse. The employer did not meet the sloping requirement for type C over type B soil that was declassified by water infiltration and equipment vibration to class C soil type and the location where employee were working was sloped steeper than 45 degree.
