Alexis Rd. Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to falls during roofing operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  Robert Wagner Roofing on or about July 20, 2015, at the address of 445 E. Alexis Rd. Toledo, OH: The employer failed to ensure employees were protected from falls while performing roofing activities atop a steep sloped portion of a commercial structure. Employees installing new roofing material from atop the structure were exposed to falls up to 18 ft. to the ground below.

Alexis Rd. Toledo, OH:  The employer did not develop or maintain a written record of certification for all employees who might be exposed to fall hazards. The employer failed to evaluate the adequacy and certify fall protection training that employees had participated through a prior employer.
