29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:    On or about July 22, 2015, the employer did not ensure an employee working from an elevated aerial lift was standing on the floor firmly.  The employee was standing on the mid-rail.  This condition exposed employees to fall hazards above 10 feet.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    On or about July 22, 2015, the employer did not ensure an employee working on a light fixture from an aerial lift was utilizing a fall protection system.  This condition exposed the employee to fall hazards above 10 feet.
