At a work site located at 6411 Precinct Line Rd., North Richland Hills, Texas; the employer did not train each employee who was performing demolition work from a scissor lift to understand, recognize and eliminate hazards such as, but not limited to, being struck by and tipping over while the lift.

At a demolition site located at 6411 Precinct Line Rd., North Richland Hills, Texas; the employer did not have an engineer perform a survey of the 16' X 50' wide wooden display wall prior to allowing demolition to take place exposing employees to struck-by hazards.
