29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   At a work site located at 6411 Precinct Line Rd., North Richland Hills, Texas; the employer did not train each employee who was performing demolition work from a scissor lift to understand, recognize and eliminate hazards such as, but not limited to, being struck by and tipping over while the lift.

29 CFR  1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations.     At a demolition site located at 6411 Precinct Line Rd., North Richland Hills, Texas; the employer did not have an engineer perform a survey of the 16' X 50' wide wooden display wall prior to allowing demolition to take place exposing employees to struck-by hazards.
