29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:         (a)  Extol of Ohio, 208 Republic Street, Norwalk, Ohio:  On or about August 31, 2015, the employer did not ensure that the unused portion of the blade on the horizontal, circular-insulation band saw was guarded. While operating the saw, employees were exposed to the unguarded blade and at risk of serious injury from hazards including but not limited to laceration hazards.       (b)  Extol of Ohio, 208 Republic Street, Norwalk, Ohio:  On or about August 31, 2015, the employer did not ensure that employees were protected from injury from the rotating drive and idler wheels and the saw blade on the horizontal, circular-insulation band saw.  The metal covers over the rotating wheels and saw blade were not secured to the frame.  Employees were exposed to rotating and moving components and at risk of serious injury from hazards including but not limited to laceration hazards.
