The employees were performing tasks such as changing the electrical disconnect box on Evaporator #10. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.119(l)(2): The procedures did not assure that all of the following considerations, including the technical basis for the proposed change, the impact of the change on safety and health, any resulting modifications to the operating procedures, the necessary time period for the change, and the authorization requirements for the proposed change were addressed prior to making any change: On or about August 26, 2015, an employee was injured while replacing an electrical disconnect box on Evaporator #10. The change was not a replacement in-kind and required a modification to the equipment (drilling new holes to secure the box). The task was part of a larger project in which the electrical disconnect boxes for all of the evaporators were to be changed out. The project was not approved through the Management of Change Procedures and the following considerations were not addressed prior to the change: (i) The technical basis for the proposed change; (ii) Impact of the change on safety and health; (iii) Modifications to operating procedures; (iv) Necessary time period for the change; and (v) Authorization requirements for the proposed change. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): On or about August 26, 2015, a maintenance employee was assigned to remove and replace electrical disconnect boxes on evaporator housings. The employee was exposed to anhydrous ammonia sustaining injuries to his face and hand. The employer did not conduct a hazard assessment prior to initiating the task to determine if hazards, such as the potential release of ammonia, were present or were likely to be present, which necessitate the use of PPE, such as, but not limited to, a face shield or a full-face respirator. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

The employer did not ensure that the employee have face protection to protect the employee's face from an accidental release of anhydrous ammonia. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about August 26, 2015, a maintenance employee was changing out an electrical disconnect box on Evaporator #10. The employee was exposed to anhydrous ammonia sustaining injuries to his face and hand. The employee was not required to: (1) Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; (2) Turn off and shut down the machine; (3) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; (4) Affix lockout or tagout devices to each energy isolating device; (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished. The employee did not isolate the machine from the anhydrous ammonia refrigerant source. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
